Sure Step Fall Assessment

Sure Step is a clinical assessment of risk for falls which is conducted in the participant's home by a Registered Nurse and a Physical Therapist. This intervention is based on evidence-based guidelines published by the American Geriatric Society, British Medical Society and American Academy of Orthopedic Surgeons.   Assessment includes evaluation of medicaitons, home environment, vision, cognition, balance, gait, and other factors.  Written recommendations are prepared for the individual and the primary care physician.  Participants also recieve monthly phone contact for a period of 12 months.  The Sure Step assessment has proven to reduce falls and use of hospital or nursing home care.
